ASPIRING MODELS UNDER THE AGE OF 18;
MUST CLICK HERE FIRST AND FIRST SUBMIT THE PARENTAL CONSENT FORM.

AGREEMENT
The model agrees that by submitting photos to Rock Brothers^TM Productions and their network of web sites, that she has all rights to such photos, and that such rights will be transferred to Rock Brothers Enterprises, Inc. I further attest that I am 18 years of age or older.

RELEASE AND HOLD HARMLESS
In consideration of my selection as a model on Rock Brothers^TM Productions network of web sites and/or any events in which I may be selected to participate, I hereby grant in perpetuity to Rock Brothers Enterprises, Inc., their agents, assigns and affiliates the use of my photograph, video, likeness, voice recording, autograph and name for all publicity and all media, including but not limited to internet, worldwide in perpetuity and commercial purposes in connection with Rock Brothers Enterprises, Inc., Rock Brothers^TM Productions, it's network of web sites, it's events wherever located and/or similar products.
